This form is for serious tattoo inquiries.

Please read all the information below before filling out the form. 

Fill out the form with your idea, references, placement, size, and if you have a budget. You can choose your preferred artists, or we can place you with an artist. Please allow us 2 business days to receive and respond to your request. 

If we need to schedule an in person consultation, we will go ahead and get that set up. We cannot schedule any appointments or guarantee any availability over email. 

If your tattoo is a cover-up please attach a clear, well lit, straight on image of what you currently have. 

A $100 non-refundable deposit is due at the time of booking and required to confirm your appointment. Tattoos quoted under $100- deposits are paid in full. 

If you are a minor you will need a government issued photo ID with the last name or address matching the parent/ legal guardian identification signing for you.
